Pvrsvcant to the provisions of Sections 6321, 6322, and 6323 of the Intemal Revenue Code of 1954,
notice is hereby given that there have been assessed under the Inte~r~al Revenue laws of the United States
agoinst the following-named tr~xpayer, taxes (incluc~ng interest and penaitiesl which after demand for
payment thereof remain unpc~id, and that by virtue of the above-mentioned statutes the amount of soid
