Tfiat he has this day applied for a detesminstion of eligibility for public
assistance fram the State of Florida under the Social ~lelfere Act, Florida Statutea,
Chapter 409, and the undersigned affiant~being the awaer of certain non-hamestead raal
property, Which ~rould mtke the affiant ineligible for public asaistaace irithout thi~
affidavit creating a lien against the real property, and the affiant hereby declares
the public asaistance payments to be msde to the affiant froai this day fos~ar~ aball
create and be a lien against the belov-deecribed real propertq for the full amount
of aaid paymenta. This lien shall be enforc ble by foreclosure against said property
at the time of the death of the undersign~d, or upon the death of the survivor of them,
whichever occurs laet, if executed by man and vife, or at such time aa affiant sells
or otherwiae alienates the land prior to his death. This lien ahall be auperior to
all other liena which do not heretofore exiet against the follawing described real
property in Saint Lucie County, Florida:
