PERSONALLY before the undersi~ned offiaer, authorized
by the laws of said State to aclminister oaths, comes FLOYD S.
RUSSSLL, v~ho upo:~ oath says that he was the husband of LADiASLOUS
RUSSSLL, ~ho died intestate on November 21, 1963, at Nassau
Hospital, Mineola, New York.
Affiant further states that during her lifetime, his
said wife was also known by the names of LADIA M. RUSSELL and
I,OTTIS RUSSELL; that he knows of his own knowl~dge that the said
Ladiaslous Russell, Ladia M. Russell and Lottie Russell, was one
and the same person.
Affiant further states that the following are the names
of the sole heirs at law of the deceased:
Floyd S. Russell, surviving spouse;
Walter Russell, James Russell and Floyd S. Russell,Jr.,
sons;
Sylvia Spargos and Mary Ann Miller, daughters;
Affiant further states that there are no deceased
children, nor any descendants of deceased children, and that
I~ there are no debts, claims or demanda,against the estate of the

said LADIASLOUS (aka Ladia M., aka ttie) RUSSELL, deceased.
