Before me, the undersigned authority, on this day personally appeared

deposea and saqs:
That,the haa thia day applied for a detesmination of eligibility for public
asaistance frem the State of Florida under the Social Welfare Act, Florida Statutea,
Chapter 409,~and the undersigned affiant being the o~mmer of certain non-hameatead real
property, which vould make the affiant ineligible for public assiatance vithout this
affidavit creating a lien against the real property, and the affiant hereby declares
the public assistance paymenta to be made to the affiant from this day forward oball
create and be a lien against the below-described real property for the full amount
of eaid payments. This lien ahall be enforceable by foreclosure against said prop~rty
at the time of the death of the undersigned, or upon the death of the survivor of them,
whichever occurs last, ff executed by man and ~rife, or at such time as affiant sells
or otherwise alienates the land prior to her death. This lien ahall be auperior to
all other liens which do not heretafore exiat against the follaiing described real

will require that the Department of Public Welfare be pai.d in full out of the proceeds
