sworn by me, the undersigned officer authorized to take acknowledgments
and administer oaths in the State and County afoxesaid, sayst
l. That the affiant is peYSOnally acquain~ed with R06A4IND
C. MONAt~AN and FRBD V. MONAHAN who were married to each othex as husband
and wife prior to August 15, 1963 when judgment was enteYSd divoYCing the
said parties and that FRBD V. MONAHAN is the same person as FRBD VINCBNr
MONAHAN as ap~ears in that certain de~d recorded in O. R. Book 7.5~, page 8~
and dated August 19, 1963 and was executed by FRBD V. MONAHAN and on whic
said date FRBD V. MONAHAN was a single adult.
2. That the affiant was personally acquainted with ANPHONY
J. WOJTsSIQ, a single adult, who held jointly certain property with LBOIA
FOX JON6S with the right of survivorship set out therein and that the
said AN1'FiONY J. WOJBSKI did die on August 26, 1969 at Nathan Littauer
Hospital, Gloversville, New York. '
