first duly sworn, upon oath says: That my name is THOMAS
A. FORBES; that Affiant is making this Affidavit under the
provisions of Florida Statute 865.09, the "Fictitious Name"
law; I am engaged in business under the fictitious name of
PITTS DRY CLEANERS, and the extent of my interest in said
business using said fictitious name is one hundred percent
(10096) .
