SURVIVORSHIP AFFIDAVIT
STATE OF FLORIDA

Before the undersigned authority personally appeared FRANK
BROOKS, who being first duly sworn says on oath that he is the
surviving spouse of Helen Brooks, deceased, who resided at 16
Louisiana Road, Lehigh Acres, Lee County, Florida, and who died
a resident of Lee County, Florida on September 23, 1972.
Affiant further says that he and Helen Brooks were married
to each other on ~,//l ~ ~ and that said marriage

continued without interruption until- the death of said Helen Brooks.
Affiant further says that he and Helen Brooka were the grantees
in that certain deed recorded in Deed ~ok 240 , page 63
of the Public Records.of St. Lucie County,~ Florida, con-
veying the following described lands:
Lot 17 of Block 5 of Unit No. 1 of that certain subdivision .
~ known as LAICEWOOD PARK, according to the map or plat thereof
on .file and recorded in the office of the Clerk of the Circuit
Court of St. Lucie County, Florida. in Plat Book 10, page t
~ 51, located in St. Lucie County, Florida.
