age 21 on October 20, 1972 , declare the State of apr residence to be the State of
Florida and execute this statement to be taken as ~r declaration of citizenship,
actual legal residence and domicile in the State of Florida.
Due to disabilities and inability to mske declaration as he 'nabove apparent,
I have never been a resident of arLy other State and first declara 'on and actuel
bona fide residence is the State of Florida where I intend to live, o reside, to
