l. The use of or cAnstruction within the right of way of the NORTH ST. LUCIE RIVER DRAINAGE
DISTRICT will be in acco?dance with the details of the approved sketch supporting this ap-
plication; and if any changes are required, same will be cleared with the DISTRICT. `
2. !f a permit is granted, I hereby agree to notify the DISTRICT at the beginning and comple- ~
tion of any c,onstruction and/or use of rigM ot way, whichever is applicable thus enabling the
DISTRIC~'to coordinate and schedule inspections.
3. If a culvert, pump, or open channe) connection is the use I propose, I accept full responsibility
, for any erosion to or shoaling in your canal or levee due to my connection and t shalt remove
or repair same promptly arid at no expense to the O[STRICT; and I wiil prevent the discharge

access of DISTRICT equipment or vehicfes without securing proper authorization therefor.
5- I further agree to alter or remove the authorized use or construction as applied for if required
in the inferests of the Nood tontro! program or as directed by any moditications to the Stan-
dards of Construction _ and Permit Procedures and at no cost to the DfSTRICT.
6. It is further understood and agreed that any other requirements of the DISTRICT are binding ,
upon me, ihe applicant and owner ,and I do he~eby indicate accepta~ce of this notice thereof. .
