Petitioner, DONNA BOYD ANDREWS, respectfully shows:
~ 1. Petitioner is the adopted daughter and one of two
aurviving heirs of C. L. BOYD, a/k/a CLARENCE L. BOYD, who died
teatate on September 4, 1972, whf.le he was domiciled at Port
Charlotte, Florida, and a resident of Charlotte County, Florida.
2. Petitioner resides at 2707 Alton Drive, Champaign,
Illinois, 61820, and her post office address is 2707 Alton Drive, -
Champaign, Illinois, 61820. ~
3. C. L. BOYD, a/k/a CLARENCE L. BOYD, died at Chapel •
Manor Nursing Home, ZO1 N.W. Olean Boulevard, Port Charlotte,
Florida, on September 4, 1972, and at the time of death was
domiciled at that address, and was seventy-four (74) years of age.
At the time of death, decedent owned an estate of approximate
. value of FORTY-FIVE THOUSAND ($45,000.00) DOLLARS.
