upon her oath, deposes and says that she is the surviving widow of
Hiram H. Van Cleve who died a residenfi of Kosciusko County, Indiana,
on January 6, 1973, at which time the undersigneci affiant and her
said husband were the owners as tenants by the entireties of the -
follo~ving-described real estate situate in Kosc~.usko. County, Indiana,

affiant further says that the marriage of this affiant and
her said husband continued uninterrupted fran'the time of their marriage
until the death of the affiant's said husband; that the affiant's said
husband made no devise or other disposition of the above-described
~ real estate; that the estate of the affiant's said husbandwas not
subject to any federal estate taxes; and that upon the death of her
said husband the undersigned affiant became the sole ocvner of the
above-described real es'tate as the surviving tenant by the entirety.
