FOR DISSOLUTION OF MARRIAGE
This cause came on for trial the 4th day of February~
1974 and the Court having given full consideration to all the
evidence, finds and determines: This Court has jurisdiction ot
, the parties and subject matter of this suit; that the relation
of husband and wife now exists between the parties; and that the
marriage between the parties is irretrievably broken and should
r be dissolved. It is thereupon

ORDERED AND ADJUDGED as follows:
~ 1. DISSOLUTION OF MARRI~ E. The marriage between
ANTKONY BLAZAK and LOIS H. BLAZAK be and the same is hereby
dissolved, and the parties and each of them are hereby forever _
freed from the bonds of matrimony heretofore existing between
and from the obligations arising therefrom.
. 2o RESTORATION OF NAME. LOIS H. BLAZAK be and she
is hereby authorized to assume her former name of LOIS H. CRUZE
and to use the same in al.l respects as if ~said marriage had not
intervened.
3. JURISDICTION. Jur~sdiction of this cause, and
the parties hereto is retained for the entry of such further ord rs
as may be proper.
