The undersigned affiant having been duly sworn~ does
say and depose the iollowings '
1. That my correct name is ERVIN BRUNSON BUSH and fur-
ther that my place of residence is 2721 South Indian River Drive~ .
Fort Pierce, St. Lucie County. Florida.
2. That I am now, and have continuously been President
of BUSH POOLS & PRECAST~ INC., a Florida corporation, headquartered
at 3309 OZeander Boulevard, Fort Pierce, St. Lucie County~ Florida,
since said corporation's inception.
3. That BUSH POOLS & PRECAST~ INC., by and through ~
signature of ~he affiant does acknowledge a corporate debt to
AI,MA M. GIST, a natural person, residing in the State of South
Carolina, in an aggregate amount of $5.700.00. and further that
said corporate debt is past due and owing.
4. That the corporate debt set forth in 3 above was
obtained solely for the establishment of the corporation, partially
by an unsecured-promissory note prior to the granting of the
Articles of Incorporation, the remainder by cash payment which was ~
received for and used in the conduct of the carporate business.
5• That each and every amount herein referred to was
` borrowed for and used'solely for the establishment and operation

of said corporation• by the affiant as Presid~ent and founder of said
~ corporation, and further that any and all interest upon borrowed

t subsequently as incurred in normal op~rations of the corporation,

7. That this Affidavit is made for the purpose of
; clarifying~past debts, acknowledging their existence and the
establishment of an inclusive agreement between the corporation

3 and said ALMA M. ~iIST for the partial retirement of said obliga-
~ tion, in order to insure continued corporate operation and
~ benefit therefrom.
