DISSOLUTION OF MARRIAGE
This cause came on for trial the~~day of ~ , 1974, ~
and the Court having given full consideration to all the evidence, finds
and determines: This Court has jurisdiction of the parties and subject
matter of this suit; that the relation of husband and wife now exists
between the parties; and that the marriage between the parties is irre-
trievably broken and should be dissolved. It is thereupon _
ORDERED AND ADJUDGED as follows:
1. DISSOLUTION OF MARRIAGE. The marriage between JAMES McELVEEN
and MARY RAY McELVEEN be and the same is hereby dissolved, and the parties

and each of them are hereby forever freed from the bonds of matrimony here-

tofore existing between them and from the obligations arising therefrom.
2. PROPERTY. The property settlement agreement admitted into
evidence is hereby incorporated herein by reference and the parties are
ordered to comply with the provisions thereof.
3. RESTORATION OF NAME. MARY RAY McELVEEN be and she is hereby
authorized to assume her former name of MARY KAY SIDAWAY and to use the
same in all respects as if said marriage had not intervened.
4. RESTRAINING ORDER. Each of the parties is hereby enjoined and
restrained from interfering with, harassing or annoying, the other.
~ 5. JURISDICTION. Jurisdiction of this cause, and the parties
~ hereta is retained for the entry of such further orders as may be proper.
