The Court further finds that tha Plaintiff would not
have provided coveraqe for the Defendant, Carol E. Sandt, had
it been apprised of her prior medical history.

IT IS ADJUDGED: ~
l. That that certain Certificate of Insurance policy

number P15-378046, be and the same is hereby cancelled as
of May 12, 1970.
2. That policy number P15 378046, as applies to the ~
Defendant, Carol E. Sandt, is of no force or effect after May
12, 1970, and that Plaintiff is not liable thereon.
3. That Plaintiff's check in the anaunt of $73.82 ~
constituting premium paid by the Defendants, plus interest,
deposited in the Registry of the Court, be paid over to the
Defendants and the Clerk of the Court is hereby directed and
ordered to remit same to Defendanta upon demand.
~ 4. That Plaintiff have judgment against Defendants
