It is, thereuFon, ~ ORDERED AND ADJUDGED as follows: 1. DISSOLUTION OF MARRIAGE. The marriage between the petitioner, PAULA M. HAVELKA, and the respondent, DAVID EDWIN HAVELKA, be and i the same is hereby dissolved, and the parties and each of them are ~ ~ hereby forever freed from the obligations arising therefrom. ~ 2. RESTORATION OF NAME. The petitioner be and she is hereby authorized to assume her prior name~of PAULA MARY DUNN and to use the same in all respects as if the marriage to respondent had not intervened. 3. RESTRAINING ORDER. Petitioner and respondent are hereby en- joined and restrained from interfering with, harassing or annoying the other. ~ 4. JURISDICTION.
