The Marriage between JOHN A, i MIKUTA and NANCY GILLIN MIKULA be and the same is hereby dissolved ~ and the parties and each of them are hereby forever freed fram the ~ bonds oi matrimony heretof ore exi~ting between them and from the ~ obligations arising therefrom. 2. RESTORATION OF NAME. NANCY GILLIN MIKUI.A be and she is hereby authorized to assume her f ormer name of NANCY M. GILLIN and to use the same in all respects as if said marriage had ttot intervened. 3. JURISDICI'ION. J~:risdictien of this cause, and the parties hereto is retained for the entr.y of such f~zrther Orders as may be proper.
