I.
~ PETITION .
• An individual entitled to receive periodic payments by
court order through the Clerk of the Circuit Court may, if without
counsel. when payments become delinquent, petition the court to
require the Respondent to paq the arrears of payments. The petition
shall be in the form of a sworn Motion for Enforcement which shall
be submitted to the Clerk of Circuit Court of the countq in which _
the original suit was filed. The Motion shall be entitled in the
original suit and must include the suit number, full name and present ,
address of Respondent, the specific amount of arrears of payanents and
the date of the order directing payments. ,

~ SERVICE
e methods of serving n on Respondent are as follows: .

1. For service by the Sheriff's Department or any autho=ized
' process server, money order or cash for service charge in the amount of

i Eight Dollars and Fifty Cents ($8.50) ($1.00 clerk's fee and $7,,S.o-
~ sheriff's fee) shall accompany the M~otion at filing.
~ - 2. For service by certified mail, money order or cash for
r service charge in the amount of One Dollar ($1.00) clerk's fee and the
correct current certified mail cost shall accompany the Motion at
filing. Money order should be made payable to the Clerk of the Circuit
Court. ~
III.
INABILITY TO PAY
Should the Petitioner state that she/he is without funds and

the fees to the Clerk, the
~ cannot obtain funds with which to pay
~
~ Support Clerk shall have the Petitioner execute an Oath of Inability
~ to Pay Fee form which shall be duly sworn to. Then the signed Oath
~
shall accompany the case file to the Hearing Judge for his approval or
~
