LAKEWOOD PARK, as per plat thereof recorded
in Plat Book 11, page 29, public records ,
of St. Lucie County, Florida.
BEFORE ME, the undersiqned authority, this day personally
appeared MARGARET MADEJ who-upon first being duly sworn, did
depose and say:
1. That she is the unreiaarried surviving spouse of
CHESTER F. MADEJ. who departed this life on August 30, 1973.
2. That she and tht~ said Chester F. Madej were the
grantees in that certain warranty deed of conveyance execute~

by Donald L. Shephard, as Trustee, and Jerome A. Simons, as
Trustee, dated November 25, 1968, recorded in O.R. Book 174,
page 2260 of the public records of St. Lucie County, Florida,
wherein the above described real property was conveyed.
3. That on the date of the aforementioned conveyance,
she was married to the said Chester F. Madej and that said
marital relationship continued without interruption until the

death of the said Chester F. Madej. ~
