FINAL DTCREE OF ADOPTION
This cause cominq before the Court on the Petition of
Norman E. Hayslip and Susanne R. Hayslip for the adoption
of the minor named above, and to legally confer upon the
child the name of Scott Edward Hayslip; and
IT p,ppEARING to the Court that notice has been given
to all proper parties to this cause and The Children's Home
Society.of Florida, a duly licensed agency of the Division
~ of Family Services of the Department of Health and
i Rehabilitative Services of the State of Florida, having
~ filed its Recom~nendation and Consent to Adoption herein;
and th~ requirements of Florida Statute, Chapter 63,
having been duly complied with; and
IT FURTHER APPEARING that Petitioners are fit and
proper persons to adopt said minor, that the child is
suitable for adoption by Petitioners, and that the bes~
interests of the child will be promoted by the adoption
prayed for by the Petitioners, and the Court having heard
~ the testimony of Petitioners that the matters contained in ~
~ the Petition are true and correct, and the Court being
~
~ fully advised in the premises, it is therefore
ORDERED and ADJUDGED as follows:
