This cause came on for trial ~he 14th day of
February, 1975 and the Court having given full consideration to al
the evidence, f~nds and determines: This Court has jurisdiction - ~
of the parties and subject matter of this suit; that the relation
of husband and wife now exists between the parties; and that the
marriage between the parties is irretrievably broken and should
be dissolved. It is thereupon _
ORDERED AND ADJUDGED as follows:
1. DISSOLUTION OF MARRIAGE. The marriage bet~,reen ANTHONY
BLAZAK and THERESA BLAZAK be and the same is hereby dissolved,
and the parties and each of them are hereby forever freed from th
bonds of matrimony heretofore existing between them and from the ~

obligations arising therefrom. ~
2. PROPERTY. The Separation and Property Settlement
Agreement admitted into evidence is hereby incorporated herein by
reference and the parties are ordered to comply ~~ith the provisio s~
