Befor2 me, the undersigned authority, personally appeared
WILLIAM F. WILLES who, being first duly sworn, says that he is
the attorney for the Lienor herein, PPG INDSUTRIES, INC. and that
Affiant's address is P. O. Box 309, Fort Pierce, Florida, and
that in pursuance of a contract with PAUL PLANTIER, INC., Lienor
furnished labor, services and materials consisting of glass, glazing
and metal and the installation thereof on the following described
real property in St. Lucie County, Florida:

12 A, as described in Official Record Book 224,
Page 2630, also as shown in Plat Book 11, Pages
35 through 37 of the Public Records of St. Lucie

owned by GENERAL HOST CORPORATION of a total value of Two Thousand ~

Two Hundred ($2,200.00) Dollars, of which there is remaining unpaid _
Zwo Thousand Two Hundred ($2,200.00) Dollars and furnished the first ~
of the same on the 13th day of November, 1974 and the Iast of the
same on the 22nd day of January, 1975 and that the Lienor served his
