BEFORE ME, the undersigned authority,
duly authorized to take acknowledgments and administer oaths, _
personally appeared CHARLES J. HARTLEY, JR., who after being
first duly sworn, deposes and says:
1. That my name is CHARLES J. HARTLEY, JR. and
I reside at 875 N. E. 74th Street, Miami, Florida.
2. That I am the son of CHARLES J. HARTLEY,
deceased, who died at sea while on the cruise ship Nordic Prince
on October 11, 1972.
3. That there is attached hereto a official copy
of the death certificate of CHARLES J. HARTLEY which was obtained
by me from the Department of Health of Puerto Rico.
4. That the body of CHARLES J. HARTLEY was returned

to Miami, Florida and was interred at Southern Memorial Cemetery
in Miami, Florida.
