KAVANAUGH, who upon her oath says: } 1. That she is the surviving spouse of EUGENE P. RAVANAUGH, who died on May 7, 1974, as shown by the certified copy of the death certificate which is attached hereto, marked Exhibit "A" and made a part hereof. 2: That at the time of the death of said EUGENE P. RAVANAUGH, . your affiant and EUGENE P. KAVANAUGH owned real property in St. Lucie County, Florida, more particularly described as: Lot 5, Block 11, Unit 2, a replat of portion of - ~ South Port St. Lucie and recorded in Plat Book 12, page 3, St. Lucie County, Florida public records. as a tenancy by the entirety. ~3y~~ DD~~ 3._ That the marriage of your affiant and EUGENE P. KAVANAUGH - was continuous and without interruption from the time of acquiring title to the real estate described above, until the death of EUGENE P. KAVANAUGH. 4. That a Federal Estate Tax Return was prepared.for the . Estate of EUGENE P. RAVANAUGH and it was determined that no Federal Estate Taxes were payable by the aforesaid Estate. Internal Revenue Service Form L-154 showing acceptance of the Federal Estate Tax Re- turn, along with Form 7990, certifying the release of your affiant from personal liability for Federal Estate Taxes, are attached here- I~ to, marked Exhibits "B" and "C" respectively and made a part hereof. ~ The affiant further states that no Florida Estate Taxes are due or ( owing and a Florida Non-Taxable Certificate No. D33845 is attached hereto, marked Exhibit "D" and made a part hereof. ~ ~ ~ ~ GRACE E.
