ARNOLD RADOFF, ("Applicant") furnishes the following in- ~

formation to thg Court for the probate of the written Will of
NINA GALLIN RADOFF ("Decedent") and for issuance of Letters
Testamentary:
1. Applicant is an individual interested in this Estate,
domiciled in and residing at 7268 Ashington Drive, Dallas,
Dallas County, Texas.
2. Decedent died on October 15, 1974 in Da.tlas, DaZZas
County, Texas, at the age of 48 years.
3. This Court has jurisdiction and venue because Decedent
was domiciled and had a fixed place of residence in this county ;

4. Decedent vwned real and personal property described_
generally as home, cash, securities~, automohiles, household
goods and personal effects of a probable value in excess of
$5000.00.
5. Decedent left a valid writte~n Will ("Wi11") dated June
11, 1971 which was never revoked and is filed herewith.
6. The subscribing witnesses to the Will and their present
residence addresses are R. Mack Lewis, Jr, 9419 Dartridge, Dallas
Texas and Carla Sheffield, 2615 Reagen, Da12as, Texas.
The will was made self proved in the L:anner prescribed by '
law.
7. Na child or children were born to or adopted by Decedent
after the dake of the Will.
8. Decedent was never divorced.
