4. RESTORATION OF MAIDEN NAME. That the Wife's.maiden
name of MAUREEN TRMIN HOLDER is hereby resto~ed to her. ;
5. ALI~ Commencing an the 7t~ day of May, 1976, f
the Husband shall pay to the Clerk of this Court the sum of ~
Twenty Dollars (~20) per week, each and every week thereafter.
In addition thereto, is hereby assessed the sum of One Dollar
(aZ) as costs_to be added to each payment made to the Clerk. !

AlI payments shall be made in cash, money order or cashier's check.
Husband and Wife are hereby ordered to keep tY~e Clerk of the Court
advised of their mailing and residence addres~:,s at all time~ while
this Order is in effect.
6. ATTORNEYtS F'~ES AND COSTS. The Court determines the
sum of ~ 3p0~ to be reasonable attorr.ey's fees for his
services herein, and the Husband is ORDERED to pay for the Wife's
attorney, said attorney's fees, together with cost herein assessed
at Thirty Dollars and Fifty Cents (~30.50)~. -
7. JURISDICTION. Jurisdiction of this cause, and the
parties hereto is retsined for the entry of such further Orders
as may bz proper.
