The undersigned. as personal representative of the above estate alleges:
1. The decedent, John A. Chutas , a r~sident of
St_ r.~c~e County Florida , died on
Augus t 18 , 19 ~ 5 , and Letters of Administration were issued to
Petitioner on Novembe r 6 9 7 5 .
2. Petitioner files herewith a final accounting containing a complete report of all receipts
and disbucsements since the commencement of administration of this estate, or since the date of the ,
last accounting Cded herein. if any. ~

3. Petitioner has fully administered this estate, by making payment, settlement, or other
i disposition of all clairr~s and debts that were presented, and by paying or making provision for the
~ payment of alt expenses of administration. ~

4. Petitioner has filed ail required estate tax returns with the lnternal Revenue Service and i
with the Departrnent of Revenue of the State of Florida, and has obtained and Cded with this court
evidence of the satisfaction of this estate's obligations for both federal and Flotida estate taxes, if any.
S. Petitioner has made or proposes to make distribudon of the assets of this estate as set ;
