detits,~including funeral expenses and last illness expenses have been
paid by the domiciliary executor.
7. The person named as beneficiary of the assets in Florida
is THEIMA RtJTH FOOTE, and it is requested that the Court order dis-
tribution accordingly:

hereinabove the value of which is $7,000.00
8. The value~of the entire estate su~b3ect to edministration

9. Attached hereto and made a part hereof are certified copies
of the death certificate of Earl M. Foote, LasC Will and Testament of
Earl M. Foote, Petition for Probate in the Estate of Earl M. Foote,
togeCher with a certificate of the Registrar of the Probate Court
in the County of Kalamazoo, SCate of Michigan,attesting to the fact
that the Will of EARL M. FOOTE was duly proved and allawed. A copy
of the funeral bill is~attached hereto and made a part hereof, said
bill having been paid by the domiciliary executor.
10. Consent form is attached hereto and made a part hereof
