The undersigned, as personal representative of the
above estate alleges: ~ -
1. The decedent, Grace L. Lee, a resident of St. ~

Lucie County, Florida, died on the 16th day of February, 1975, ~
and Letters of Administration were issued to Petitioner on ~
February 27, 1975. .

~ 2. Petitioner files here~with a Waiver of Account and
! Service of Petition for Discharge and Consent to Discharge
of Personal Representative.
3. Petitioner has fully administered this estate,
by making payment, settlement, or other dispostion of all _
claims and debts that were presented, and by paying or making
provision for the payment of all expenses of administration.
4. Petitioner has filed all required estate tax re-
turns with the Internal Revenue Service and with the Depart-
ment of Revenue of the State of Florida, and has obtained and ,
filed with this court evidence of the satisfaction of this

II estate's obligations for both federal and Florida estate
taxes , if any . ~
5. Petitioner has made distribution of all of the

assets of this estate.
