BEFORE ME, the undersigned authority, personally appeared t

MRS. RUTH ANN ZOOK, who, being duly sworn, says that she is the ~
agent of the lienor herein, PALMER HOUSE SERVICES, whose address
is 10800 North Military Trail, Palm Beach Gardens, Florida 33410,
and that in pursuance of a contract with PAUL W. DAVIS SYSTEMS,
of 113 Palmetto Lane, Fort Pierce, Florida 33450, lienor furnished
labor, services, and/or materials consisting of the scraping of all
floors, including the removal of plaster, the installation of
carpets and pads, the preparation of two (2) bathroom floors for
vinyl, and the installation of vinyl floors therein, on the follow-
ing described real property in St. Lucie County, Florida:
402 N.E. Solida Circle, Port~St. Lucie,
Florida, which is leqally described as
FLORESTA PINES UNIT TWO, Block 3227, Lot 29,
owned by Colonel Richard E. Campbell, of a total value of One
Thousand Two Hundred Ninety-Four and 25/100 ($1,294.25) Dollars,
of which there remains unpaid One Thousand Two Hundred Ninety-
Four and 25/100 ($1,294.25) Dollars, and furnished the first of

on or about February 19, 1977, and that the lienor served its .
