THIS CAUSE having come on for hearing on Petition of .
Department of Health and Rehabilitative Servicea of the Sfiate of
Florida for Child Support and the Court having found that said
Petitioner is a proper party by virtue of an assignment of rights
to child aupport aigned bq the~party having custody of the dependent ~
child~and all parties having received proper notice, it is _
ORDERED AND ADJUDGED that the Petition~ ahall be and the -
8ame is hereby granted; it is further
ORDERED AND ADJUDGED that commencing on September 2, 1977,
the responsible parent WILLIE J. WILLIAMS, who is the father of the
~ child herein, ahall pay to the Clerk of this Court the sum_of $31.00
per week each and everq week fran the first week of September through
the last week of June of each year, which amount shall be remi~ted
monthly bq the Clerk of the Department of Health and Rehabilitative
Servicea, Child Support Enforcement Unit, 1317 Winewood Boulevard,
Tallahaseee, Florida, 32304. In addition thereto the sum of ~1.00
as costa shall be added to each such payment made to the Clerk. All
paymenta shall be made in cash, moneq order or cashiers check. Al1
money ordera and cashiers~checka shall bear the payees name and
Social Security number and shall be made payable to the CLERK OF THE
CIRC[IIT COURT, and sent to : -
