petitions this Court for approval of settlement and authorization
to execute appropriate releases ar other do~uments relative to
~ claa.m under the Florida Wrongful Death Act, Florida Statute 768.16 {
et seq., for the death of JEFFREY LEO MILLER, against JIMMIE
CLAYBOURN SMITH, SR., W. C. GRAVES, JR. and/or GRAVFS & SONS
GROVES, IlIC., and/or INDIAN RIVER ASSOCIATION, II~TC., and FLORIDA
FARM BUREAU MUTUAL INSURANCE COMPANY, and for an Order of summary' ~

administration, and would show unto the Court as follows: `

1._ On August 14, 1977, JEFFREY LEO MILLER was killed as
~ a result of an acciden~: occurring on Peterson Road, said road
~
deadends into Jenkins Road, St. Lucie County, Florida, at ~

approximately 7:15 p.m., wherein JEFFREY LEO MILLER was a ~

passenger in the 1972 Fard pickup truck owned by W. C.GRAVES, JR.

TION, INC., and operated by JIMRiIE CI~AYBOURN SMITB, SR., from
_ which the decedent was accidentally bumped by another minor,
resulting in his falling out of said vehicle. '

~ 2. At the time of JEFFREY LEO MILLER's death, he was ~
survived by the Petitioner herein, BERNICE BROWN ISAIAH, his
mother, who has a claim pursuant to Chapter 768, Florida
