THIS CAUSE having come on for hearing on Petition of the
Department of Health and Rehabilitative Services for Child Support

• and the Court having found that said Petitioner is a proper party
by virtve of an assigmiaent of rights to child support signed by
the party having custody of the dependent child~ren) and all parties
having received proper notice, it is

the Department of Health and Rehabilitative Services, Child Support
Enforcement Unit, 1317 Winewood Blvd., Tallahassee, Florida, 32304.
~ In addition thereto the sum of $2.00 as costs shall be added to each
such payment made to the Clerk. All payments shall be made in cash,

v money order or cashiers check. All money orders and cashiers checks
