3. In additlon to the regular cash support payments as specified
above, the Husband is hereby required to keep in full force and effect, a
policy of inedical and dental insurance for the benefit of the minor child
of the parties. The Husband shall provide the Wife with all necessary claim
forms, proof of insurance a~d other inforniation necessary to file prompt
claims with the lnsurance carrier. The Wife shall promptly file all insurance
claims and shall provide all necessary docunentation and other information ~
i
required by the insurance carrier. In the event either party advances payment
of inedical or dental costs covered by the sub~ect i~isurance policy and beneflts
are ultimately recovered under said policy, the individual party originally ~
advancing payment of such medical or dental costs, shall be reimbursed from
the benefits recovered under the claim. .
4. The Wife shall keep the Husband infornied of the medical and ~
dental condition of the minor child of the parties. In the event the Nusband ~

desires regular 60 day reports, he shall inform the Wife of his desires in ~
writing and the Wife shall comply by making regular written reports as to
the minor child's condition to the Husband. In addition, the Mife shall
immediately notify the Husband by telephone or other prompt communication, .
of any emergency conditions which arise with respect to the minor child's

condition. '~In the event the Nusband desires to have the minor child examined
~ by another physician or denttst, he shall be entitled to have the child so

examined by a local physician or dentist located in either Martin County, -

St. Lucie County, Indian River County or Okeechobee County, Florida, and

; shall be responsible for making the arrangeme s :

shall also be responsible for the payment of same. I

5. If the Husband desires to communicate with the Wife in the '
~ future, he shall do so by letter or telegram and shall not telephone the
Wife, except in emergencies. t

6. The right of the Husband to reasonable visitation rights with ~
~ the minor child of the parties is hereby confirn~ed, however, such right

~ of reasonable visitation shall be exercised sub~ect to the following guide-

~ 1. The Husband shall provide the Wife with reasonable notice of his

~ intention to visit with the child, which notice shall not be less than 15 days
~
~ and shall be in writing.
