THIS CAUSE came on for trial the ay of {

1978 arid the Court having given full consideration to all the ~

evidence, finds and determines: This Court has jurisdiction ~
of the parties and subject matter of this suit; that the rela- ~
tion of husband and wife now exists between the parties; and ~
~
that the marriage between the parties is irretrievably broken ~
and should be dissolved. It is thereupon t
~
ORDERED AND ADJUDGED as follows: ~
1. DISSOLUTION OF MARRIAGE. The marriage between RICHARD
VOTH and DEBRA JEAN VOTtI be and the same is hereby dissolved, ~
and the parties and each of them are hereby forever freed from
the bonds of matri~ny heretofore existing between them and
from the obligations arising therefrom. ~
2. RESTORATION OF NAME. DEBRA-JEAN VOTH be and she is ~
~
hereby authorized to assume her former name of DEBRA JEAN COX ~

and to use the same in aIl respects as if said marriage had not
intervened.
3. RESTRAINZNG ORDER. Each of the parties is hereby
enjoined and restrained from interfering with, harassing or
. annoying, the o~her.
