THIS CAUSE having came on for hearing on the petition of the
Department of Hea1Ch and Rehabilitative Services for ~hild support and.
the Court having found that said Petitioner is a proper party by virtue
of an assignment of rights to child support signed by the party_having
custody of the dependent child(ren) and all parties having received
proper notice, it is ~
ORDER,ED AND ADJUDGED that the Petition for Child SupporC
shall be and it is hereby granted; it is further ~
ORDEP•ED Al~'D ADJUDGED that comaencing on /1'I-c~-~~~=~i Z 7 ,

who is the ~~c. T h~ of the child(ren) herein, shall pay to the

ment Uzit, 1317 Winewood Blvd., Tallahassee, Florida, 32304_ In

payment made to the Clerk. All paynents shall be aade in cash, money
; order or cashiers check. All noney orders zrui cashiers checks shall
~ bear the payees name and Social Security Nunber and shall be made payable
~ to the CLERK OF THE CIRCUIT COURT, and sent to:

ORDERED AND ADJUDGED that the Clerk of this Court shall and
he is hereby ordered to continue to transmit support paya!ents received
frors the responsible parent to the Department of Health and RehabilitativE-
Services until further order of this Court to the coatrary or receipt
of 1`:o~ice to Di.scontinue Payments from the Department of Health and
~ Rehabilitative Services, i.n which event the support payments shall
thereafter be directed and payable to the person having custody of the
chiZc(ren). . _
