THIS CAUSE having come on for hearing on the petition of the
Department of Health and Rehabilitative Services for child support and
the Court having found Chat said Petitioner is a proper party by virtue
of an assignment of rights to child support signed by the party having
custody of the dependent child(ren) and all parties having received
prope~ notice, it is
ORDERED AND ADJUDGED that the Petition for Child Support
shall be and it is hereby granted; it is further

~ bear the payees name and Social Security Number and shall be made payable
~ to the CLERK OF THE CIRCUIT COURT, and sent to:

he is hereby ordered to continue to transmit support payments received
from the responsible parent to the Departnent of Health and Rehabilitativ
Services until further order of this Court to the contrary or receipt
~ of Notice to Discontinue Payments from the Department of Health and
~ Rehabilitative Services, in which event the support payments shall
~ thereafter be directed and payable to the person havin~ custody of the
ch~l~(ren). ~
