The Petition of PATRICYA HAUPIN and NORMAN MAUPIN, respect-
fully show:

1. Petitioners reside at 89 Maple Drive, Okeechobee, Florida. .

address are as follows: TONYA LESHAWN MAUPIN, age 1-1/2 years, date of ;
birth, 12/26/76, 89 Maple Drive, Okeechobee, Florida and is related to
Petitioners as natural child; Petitioners have custody of their child ;

3. TONYA LESHAWN MAUPIN is the owner of real and personal
property consisting of a cause of action against Antonio Hernandez-
~ Garcia, M.D. which is currently pending in the Civil Division of this

Court as Case No. MLMC 78-28 CA. Approval of settlement of said case
- was rendered in Chambers by Judge Charles E. Smith on ,
1978, a true copy of which is attached hereto and made a part hereof.
That which will inure to the benefit of the minor and the purpose of this
guardianship will be in excess of $5,000.00 after costs and attorneys'
