~ This cause, having come on before the Court on the
. ~
motion of Defendant, Dorothy R. Panzone, for summary judgment, '
and the Court having reviewed pertinent pleadings, the
deposition ot Donald V. May, and the affidavit of Dorothy R. .
Panzone Lemish, and having received argument of counsel and
being otherwise fully advised in the premises, finds as
follows:
1. John L. Panzone was the named insured under Coastal
~ States Life Insurance Company group policy #1-5819 at the
time of his death on August 13, 1976.
~ Z. That the three most recent applications for increased
coverage under said policy of insurance name defendant,
Dorothy R. Panzone, as the beneficiary.
3. That each subsequent application superseded prior
apglications and became a part of the po2icy of insurance. -
4. That Dorothy R. Panzone is now known as Dorothy R.
Panzone Lemish. -
WHEREFORE,-it is Adjudged: - -
1. Dorothy R. Panzone Lemish is the beneficiary under
Coastal States Life Insuran~e Company GrQUp Policy ~1-5819
on the life of John 3,. Panzone, deceased. -
