Wagner, Nugent, Johnson & McKeown, P,A. amounting to one-third of the
settlement proposed and additionally has incurred costs for her attor-
neys in the sum of $407.33, and Petitioner requests allowance for
attorneys' fees of $3,333.33 and costs in the amount of $407.33 which
result in the net amount available for distrrbution to ~the survivor
under Florida Statute ~768.16 et seq. of $6,259.34.
6. There are no unpaid medical bills remaining for services
rendered to decedent from the time of her accident to the time of her
death and there are also no unpaid funeral service bills.
WHEREFORE, your Petitioner requests an Order approving
settlement of the claim and the terms as set forth herein and further
requests that the Court order approval and payment of the attorneys'
fees and costs under the £oregoing terms and authorize the disbursement
of the remaining sums of settlement to Timothy Wayne Adams, who has
since attained the age of majority, as survivor, pursuant to the
Florida Wrongful Death Statute.
