The undersigned, as personal representative of the above estate alleges:

Petitioner on July 20th , 19 7$ ,
2. Petitioner fdes herewith a Cuia! accounting containing a complete report of all receipts
and disbursements since the commencement of administration of this estate, or since the date of the
last accounting Cded herein, if any.
3. Petitioner has fully administered this estate, by making payment, settlement, or other
disposition of all clairns and debts that were presented, and by paying or making provision for the
payment of all expenses of administration.
4. Petitioner has filed all reyuired estate tax retums with the Internal Revenue Service and
{ with the Department of Revenue of the State of Florida, and has obtained and fded with this court
evidence of the satisfaction of this estate's obligations for both federal and Florida estate taxes, if any.
