~ The Petition of MARY F. DAVIS, as the duly acting and
~ .
~ qualified Ancillary Personal Representative of the Estate of THOMAS
JOSEPH KAVENY, deceased, shows respectively;
! 1. THOMAS JOSEPH KAVENY died October 4, 1976, and on April
~ 20, 19??, the Petitioner was appointed Ancillary Personal Represen-
~ tative and continuousl has since that time dischar ed the duties

~ of Personal Representative as required by law.

~ 2. Petitioner has published a Notice to Creditors of the
I estate as required by law, the first publication of which occurred
I
i on July 18, I978, and Proof of Publication of that Notice has been
I
( filed. The period during which claims can be filed validly against
~
the estate has expired. There were no claims filed in connection
with this estate~
~ 3. The decedent's gross estate for tax purposes was more
~
: than $60,000.00. There was tax due the Federal Government in
(
~ connection with this estate, and your Petitioner has obtained and
filed with the court a Final Certificate from the State of Florida,
being No. A 17765, and a final Closing Letter from Internal Revenue
Service.
+ 4. Petitioner states that she is the sole beneficiary of
the Estate of THOMAS JOSEPH KAVENY, and attaches her $eceipt for
all Florida assets due her as sole beneficiary of this estate.
5. The Petitioner has gathered all of the assets of the
estate that are collectible and has otherwise faithfully adminis-
tered all the goods, chattels, rights and credits of the decedent.
6. Petitioner states that she has made a total distributio
of the Florida assets pursuant to the Will of THOMAS JOSEPH KAVENY.
7. Nothing remains to be done for the completion of the
administration of this estate except payment of attorney's fees and
