The•undersigned does hereby apply to the Fort Pierce
Utilities Aufhority for an extension of water service outside the City
limits of the City of Fort Pierce, Florida to the following described
property which I oan. to-wit:
Lot 14, Blocic 7, Airport Industrial Park ~
The mailing address of the above described property is
Industrial Avenue 3, Fort Pierce, Florida
I understand that in order to obtain extension of such
water service that I will be required to pay the cost of construction
of such water line from the point of connection wfth existing lines of
the Utilities Authority to my property, that the total of such cost
sha11 be deposited with the lltilities Authority prior to the time of
the constructian of said line; that I Will be obligated to pay all
meter fees and connection charges required under the Resolutions of the
Utilities Authority for such service prior to the installation of said
line; that I will pay, when billed, for all water furnished to my
property according to the rates established by the Utilities Authorfty
for the service I receive.
I understand that the extension of said water service to
my property shall be contingent upon the ability of the Utilities
Authority to supply the demand requested by me without in any way
~ interfering with the water services now performed by the Utilities
ci ,Author~ty for the inhabitants of the~City df Fort Pierce, Floric~a.

` I hereby agree that in consideration of my request for
---the extension of water service to my property under the conditions above
;_mentioned that if my property becomes contiguous to the City limits of
::'the City of Fort Pierce, Florida as such term is defined in the laws of
_ 1-Florida, then I shall immediately apply for the annexation of my property
T to the City of Fort Pierce, Florida.

I further agree that this understanding and agreement
_~shall be binding upon my heirs, successors or assigns and that this
document may be recorded in the publ.ic records of St. Lucie County,
Florida for the purpose of serving as notice of the terms of this
agreement.
