Dear Sir:
The undersigned does hereby apply to the Fort Pierce
Utilities Authority for an extension of Water service outside the City
limits of the City of Fort Pierce, Florida to the following described
property which I own. to-wiC:
Lot 10, Block 30, Sunland Gardens, rt. Pierce, Florida
The mailing address of the above described property is
3901 Avenue~M, Ft. Pierce, Florida 33450
I understand that in order to obtain extension of such
water service that I will be required to pay the cost of construction
of such water line from the point of connection with existing lines of
the Utilities Authority to my property, that the total of such cost
shall be deposited with the Utilities Authority prfor to the time of
the construction of said line; that I will be obligated to pay all
meter fees and connection charges required under the Resolutions of the
Utilities Authority for such service prior to the installation of said
line; that I will pay, when billed, for all water furnished to my
property according to the rates established by the Utilities Authority
for the service I receive.

I understand that the extension of said water service to
my property shall be contingent upon the ability of the Utilities

Authority to supply the demand requested by me without in any way
•o, ~ interfering with the water services now performed by the Utilities
° Authority for the inhabitants of the City of Fort Pierce, Florida.

I hereby agree that in consideration of my request for
~ the extension of water service to my property under the conditions above
~a. mentioned that if my property becomes contiguous to the City limits of
~ ~ the City of Fort Pierce, Florida as such term is defined in the laws of
j - Florida, then I shall immediately apply for the annexation of my property
p ~ to the City of Fort Pierce, Florida.

` shall be binding upon my heirs, successors or assigns and that this
