PETITION FOR DISCHARGE
The undersigned, as personal representative of the above
estate alleges:
l, The decedent, ANNA KRA,TICEK, a resident of St. Lucie
County, Florida, died on May 2., 1978, and Letters of Administra-
tion were issued to the Petitioner on May 8, 1978.
2. Petitioner files herewith a waiver of a final, or other,
accounting, executed by the Petitioner as the sole beneficiary
of the estate of the decedent.
3. Petitioner has.fully administered this estate, by

~ making payment, settlement~ or.other dispositioti of all claims
~
4 and debts that were presented, and by paying or making provision
` for the payment of all expenses of administration.

~ 4. Petitioner has filed all required estate tax returns

R with the Internal Revenue Service and with the Department of

~ Revenue of the State of Florida, a copy of which is attached
hereto and incorporated herein by reference, showing that there
~ is no estate tax due or owing on the estate of the decedent. ~
~ 5. Petitioner has made or proposes to make distribution ,
~ of all of the assets of this, estate to himself, as sole bene-
~ ficiary of the estate of the decedent. The only person having
an interest in this proceeding and his address is JOSEYri
~ KRAJICEK, 201 S. 22nd Street, Fort Pierce. St. Lucie County,
Florida, surviving spouse of the decedent.
