THIS CAUSE having come on for hearing on the petition fo the DEPARTMENT
OF HEALTH AND REtiABILITATIVE SERVICES for child support and the Court having found
that said Petitioner is a proper party by virtue of an assignment of rights to
child support signed by the party having custody of the dependent child(ren) and
all parties having received proper notice, it is
ORDERID AND ADJUDGED that the Petition for Child Support shall be and
it is hereby granted; it is further l
ORDERID AND ADJUDGED that commencing on ~ , 1978,

the responsible parent CHARLES SMITH , who is the FATHER
of the minor child(ren), to-wit: ANGEL SMITN D.O.B. 1-8-63

ORDERED AND ADJUDGID that the Clerk of the Circuit Court shall and (s)he
~ is hereby ordered to continue to transn~it support payments received from the Respondent
~ to the Department of Health ar.d Rehabilitative Services until further order of this
~ Court to the contrary or receipt of Noiice to Discontinue Payments from the Department ~
of Health and Rehabilitative Services, in which event the snpport payments shall there-
~ after be directed and payable to the person having custody of the child(ren).
