Dear Sir:
. The uridersigned does hereby appl}• to the Fort Pierce
=Utilities Authority for an extension of water service outside the City
limits of the City of Fort Pierce, Florida to the following described
properiy which I own, [o-wit:
Lot 8& W~ of Lot 7, Blk. 37 •
Page 67, Plat Book 9, St. Lucie Co. Fla. • '
The mailing address of the above descrihed property is
3605 Avenue M
I understand that in order to obtain extension of such
water service that I will be required to pay the cest of construction
of such water line from the point of connection with existing lines-of
the Utilities Authority to my property, that the total of such cost
shall be deposited with the Utilities Authority prior to the time of
the construction of said line; that I will be obligated to pay a12
meter fees and connection charges required under the Resolutions of the
Utilities Authority for such service prior to the installation of said
2ine; that I will pay, when billed, for all water furnished to my
property according to the rates established by the Utilities Authority
' for the service I receive.
; I und~rstand that the extension of said water service to
' my property shall be contingent upon the ability of the Utilities
i Authority to supply the denand requested by me without in any way
~ interfering with the water services now performed by the UtiZities
~ Authority for the inhabitants of the City of Fort Pierce, Florida.

f I hereby agree that in consideration of my request for
the extension of water service to my property under the conditions above
' mentioned that if my property becomes contiguous to the City limits of
the City of Fort Pierce, Florida as such term is defined in the'laws of
Florida, then I shall immediately apply for the.annexation of mq property
to the City of Fort Pierce, F1oTida. .
I further agree that this und~rstanding and agreement
shall be binding upon my heirs, successors or assigns and tnat this
document may be recorded in the public records of St. Lucie County,
Florida for the purpose of serving as notice of the terms of this
agreement.
