_ The undersigned does.hereby apply to the Fori Pierce
= Utilities Authority for an extension of water servi~e outside the City
limi[s of the City of Fort Pierce, Florida to the following described
property which I own, to-wit:
Lot 15, Blk 25, Plat Book 8, Page 32 •
St. Lucie Co.
The mailing address of the above described property is
3908 Avenue M
I~nderstand that in order to obtain extension of such
water service that I will be required to pay the cost of construction
of such water line from the point of connection with existing lines of
the Utilities Authority to my property, that the total of such cost
shall be deposited With the lltilities Authority prior to the time of
the construction of said line; that I~ill be obligated to pay all
meter fees and connection charges required under the Resol~itions of the
Utilities Authority for such service prior to the installation of said
line; that I will pay, when billed, for all water furnished to my
property according to the rates established by the Utilities Authority
for the service I receive.

_ I understand that the extension of said water service to
my property shall be contingent upon the abilit} of the Utilities ~
Authority to supply the demand requested by me without in any way
interfering with the water services now perfot~med by the Utilities
; Authority for the inhabitants of the City of Fort Pierce, Florida.

' I hereby agree that in consideration of my request for
the extension of water service to my ro ert under the conditions above

mentioned that if my property becomes contiguous to the City limits of
the Cit~• of Fort Pierce, Florida as such term is defined in the laws of
Florida, then I shall immediately apply fo.r the annexation of my property
to the City of Fort Pierce, Florida.
Z further agree Lhat this ur.derstanding and agreement
shall be binding upon my heirs, successors or assigns and that this
document may be recorded in the public records of St. Lucie County,
Florida for the purpose of serving as notice of the terms of this
agreement.
