()(b) The total life insurance ~remiuns which would have
~ been paid to date of death on any insurance with
respect to which the Participant was entitled under
the Plan but which insurance was not pucchased by
~ the Trustee for reasons of uninsurability oc sub-
standard extra rating.
()(c) The actuarial value of the retire~ent annuity ac-
crued to the date of death. ~
- ()(d) The return of premiuns ~aid or, if greater, the ~
cash value oE any~level premiun annuity contract
purchased to fund the retirenent benefit of a Par-
~ ~ ticipant. In no event shall the total death benefit _
thus provided exceed the actuarial value of the
retiranent anriuity accrued to the date of death.
