ARTICLE IV
EMPLOYE~S ENTITLED TO PARTICIPAT~
IV:1 Employees of the Employer eligible to become parti- ~
cipants unde~ this Plan and Trust shall be all those
present and Euture employees of the Employer, as
defined in Section II:1 (d) of Article II, •Defini-
tions of Terms'. who,_ as_ of the Effective Date. or,
if Iater, an Entry Date:
(a) Are employed and actively enyayed in the _
conc~uct of the business of the Employer and ~
not ~n a leave of absence or in the Armed
Forces of the United States or any of its ;
Allies. Leaves of absence shall be applied ;
uniformly for all employees. ;

(b) Have satisEied the eligitility requirements ;
s ecified by the EmploYer i_ n Item 7 in the '
opt ion AcZreement . ~ -

(c) Shall make and file wiCh the Comr~ittee,
within thirty (30) days after the receipt of
i~ _ the form, a written application on the form
provided by the Committee for th~t purpose
and shall.by provision in such form in all
respects aqree to abide by the provisions of
this plan and Trust; and shall in such form
ac~ree: ~ ~
(1) To cxe~:ute such instruments and su5mit
t~ s~ch physical examinations as may be
rec~uired by the Insurer in-connection
with an apnlication for a contract on
his life, and
(2) To answer truthfully and completely, _
without reservation or concealment any
question or request for information by
the Insurer in connection with~the is-
suance of such contracts on his life. ~
(3) To contribute to the cost of the Plan ~
each month from date of entry to re- ~
tirement date the amount s ecified b,~Y ~
the ~mploYer in Item 8 in t e A opt[on ~
Agreement. ~
